Media Mail and Insurance Coverage.

(Original size is approximately 11" x9 1/4")

This package was mailed at book rate with insurance coverage from Lake Zurich, Illinois, (60047) to Pomeroy, Washington (99347).

I am a bit confused by the charges on this.

To receive a numbered insurance slip, the insurance coverage must be above $50.00 at a minimum cost of $2.20 for $50.01 to 100.00. (The item in the box was valued at $49.95 and weighs 2 1/4 pounds). This leaves $2.63 for postage. Postage for a parcel < 3 pounds is $2.26  (overpayment of 37 cents) and for under 4 pounds is 2.68 leaving a balance due of 5 cents.

With an insurance coverage of $.01 to 50.00, the fee is $1.30. I'll assume that the numbered insurance form was used by mistake. This leaves $3.53 for postage, with the Media Mail rate for >5, but <6 pounds  being $3.52 resulting in a slight overpayment of 1 cent.
